How can I register?

 
  1. The Contract and the Application Form are printed (two copies);
  2. The application form and the contract are filled in and signed (if the payment is done by a legal entity, the contract is stamped);
  3. For completing the application are required: copy of identity card; copy of birth certificate; copy of name change (if applicable); copies of the documents attesting latest studies completed;
  4. Mandatory is to download and study CODECS Regulations;
  5. The documents are then registered at CODECS (pay attention to the registration end-date);

Supply Chain Innovation, Strategy and Management Program fee is 1249 Euro / student + VAT (full payment) and includes the following: printed materials - two volumes course namely: Vol.1. Module I "Of what consists the supply chain and distribution" and Vol 2. Module I "Consistency of supply and distribution chain", Module 2 "Inter-organizational dynamics", Module 3. "Policy and practice in the organization" Module 4 "New models and competing models", Collection of articles - taken from the book "Supply and distribution chains and total product systems", edited by E. Rhodes, Warren J. and Carter R., Blackwell, 2006, 4 DVDs: "Introduction to the supply and distribution chains", "Retail", "Product systems", "Support health Services" and Glossary of the used terms, costs of instruction teaching costs: 3 tutorials and Residential School (accommodation and meals, transportation costs to Residential School are not included).
